What Is an Anal Fissure (Pittaja arshas)?
Anal fissure is a small linear ulcer in lower part of anal canal. It is tear occurred due hard stool. The symptoms are severe burning pain during and after defecation with blood. Patient feels intense fear for bowel movement (defecation), hence try to avoid defecation. It is confused commonly with haemorrhoids.
What causes an anal fissure?
Causes of fissure- Primary fissure- constipation
- Secondary fissure- other diseases
Primary fissure is due to constipation
Primary fissure is generally caused by trauma due to constipated hard stool. A hard and dry bowel movement is typically responsible, but loose stools or diarrhoea can aggravate fissure. The trauma produces severe anal pain, resulting in anal spasm, which results to decrease in blood flow to the anal mucosa. This impairs the healing of fissure.
Secondary fissure is due to other diseases
Secondary fissure occur due to others diseases. It is due to diarrhea, inflammatory bowel disease, Crohn’s disease, ulcerative colitis, syphilis, tuberculosis, leukemia, HIV/AIDS, or rectal cancer. These diseases cause atypical fissures which will have multiple fissures. In these conditions, we have treat the primary cause of disease and then treat fissure.
What is the treatment of anal fissures?
Types or stages of fissure
- Acute fissure
- Chronic fissure
- Complicated fissure
According to the stage treatment protocol will be differ. The 90% of anal fissures do not require surgery. The most common treatment for anal fissure:
- Stool softeners or bulk laxative
- Local application anal ointments
- Worm water Sitzbath
- Balanced diet
- More fruits and water
- Minimum four kilometres walk
- 7 hours regular sleep
In acute fissure, with above advises plus, deepana, pachana, vatanulomana drugs and localised matra vasti (medicated oil enema) are useful treatment.
In chronic fissure, patient may need dilation procedure with above medication. If the patient is not responded, advise sphincterolysis procedure based on the patient condition.
The complicated fissures are the hypertrophied anal papillae and low anal intersphincteric abscess. These two conditions need surgical intervention.
