What are the Haemorrhoids(Piles, Arshas)?

Piles are a well-known common disease since thousands of years. The common symptoms are bleeding, prolapse and pain. Most of the population have had experienced some symptoms related to piles in lifetime.

Haemorrhoids or piles are the distended veins in the anal canal or surrounding anus. When veins in the lower part of rectum or anus stretch under pressure, they swell and prolapse from anus.

Types piles (piles, arshas) internal and external

Internal Haemorrhoids- ardra arshas or raktaja arshas

The haemorrhoidal veins in anal columns and rectum are distended resulting into haemorrhoids. The common symptoms are painless bleeding, prolapse, itching, and mucus discharge.

According to the prolapse, the haemorrhoids are classified into four degrees.

• Grade 1: Nil prolapse
• Grade 2: Prolapse and goes back on its own
• Grade 3: Prolapse and pushed back by patient
• Grade 4: Prolapse and stay outside

External pile- suska arshas

The set of veins under the skin around the anus are distended resulting in piles. The Symptoms are presence of lump just outside the anus.

Thrombosed external pile is an acute painful pile near the anus. It is a blood clot develops due prolonged straining at stool, long sitting or driving.

Treatment

Acharya Susrutha explained four types of procedures to treat haemorrhoids.

• Medical treatment
• Kshara application- for first and second degree haemorrhoids
• Agnichikista- excision of haemorrhoids by using thermal energy
• Sastra chikista- surgical haemorroidectomy

We are using above methods according to the degree of haemorrhoids and patient condition under local anaesthesia.

Contributing factors
